Do You Qualify?

To receive Medicare home health care, you must meet all four of the following criteria set by the Centers for Medicare & Medicaid Services (CMS):

1.You are considered homebound. it is difficult or tiring for you to leave home without assistance.
2.You require intermittent skilled nursing care, physical therapy, speech-language pathology, or continued occupational therapy.
3.You are under the care of a physician, nurse practitioner, or clinical nurse specialist who certifies your need for home health care.
4.You receive care from a Medicare-certified home health agency.

What Does It Cost?
